Eligibility
Health District services generally are intended for people who live within the Health District service area in northern Larimer County, Colorado.
View the Health District boundary map to look up your address and determine whether you are a resident of the Health District.
Some Health District services are open only to residents of the Health District whose household income is below a certain level. These include services provided by Prescription Assistance and the Family Dental Clinic, as well as reduced-cost counseling arranged through Mental Health Connections.
Other Health District services are available to all residents regardless of income. These include cholesterol testing and quit tobacco services. These services also have sliding fees for those who have low incomes. Individuals who live outside the Health District may be able to use these services on a space-available basis as long as they pay full price. People living outside the Health District boundaries generally are not eligible for any sliding fees or reduced-cost services.
